Care Coordination Co-design: COORDINARE

Client: COORDINARE - South Eastern NSW Primary Health Network

Background

The purpose of the Care Coordination Co-design Project was to rapidly research, co-design and frame a commissioning intent for a care coordination model of care for people with chronic and complex health conditions in South Eastern NSW PHN. 

COORDINARE engaged Beacon Strategies to research and co-design a model of care that clearly meets the needs of consumers and providers across the region and supports the optimal delivery of care for people with chronic and complex health conditions in the community setting.

Approach

We used the following approach to deliver the project:

1. Evidence review

The evidence review analysed documents supplied by the client, reviewing and synthesising existing evidence. Independently sourced documents relating to similar models in the Australian context were also reviewed. This phase allowed model options to be proposed, opportunities for local enhancement and commissioning considerations. 

2. Stakeholder engagement

Beacon Strategies facilitated and summarised the outcomes of a half-day round table discussion where key regional stakeholders provided insight regarding the proposed model of care and how it would be best localised for implementation. The half-day round table focused on:

  • Capturing the views and insights of both consumers and health professionals

  • Reviewing the proposed models of care and providing insight on what would work and what wouldn’t in meeting the local needs of the community.

  • Rapidly designing elements of the service model where gaps in information may exist in the literature

3. Summary report

Following the completion of the stakeholder engagement phase, the project team prepared a suite of reporting products for submission to COORDINARE. 

Outcomes

This project was closed out through the delivery of a final report to the COORDINARE team. The contents of this report highlighted:

  • The situation and context for the care coordination model

  • The rationale for a care coordination model for people with chronic and complex health conditions

  • What COORDINARE’s care coordination model should look like and, 

  • The next steps in commissioning the model
