Commissioning early intervention initiatives for Brisbane North PHN

Background

To inform the commissioning of early intervention initiatives to support healthy ageing and ongoing management of chronic conditions, Brisbane North PHN (BNPHN) engaged Beacon Strategies to undertake an intensive desktop analysis of the strategic environment, population health needs, evidence base and service mapping relating to healthy ageing initiatives for the Brisbane North region. 

The project also had a specific focus on older adults at risk of poor health outcomes such as Aboriginal and Torres Strait Islander peoples, people from Culturally and Linguistically Diverse (CALD) backgrounds, people in the LGBTIQ+ community, people who are homeless or at risk of being homeless, and those who are socially isolated.

Approach

Our project team approached the project via a series of phases. Project phases include;

  • Phase 1: project establishment — developed key project management documentation, enabling visibility of project approach and scheduled activities. 

  • Phase 2: desktop research (needs) — reviewed existing Brisbane North PHN documentation and scanned available literature relating to the needs of older adults aligned with the target groups of the project.

  • Phase 3: data analysis (needs) — analysed population demographics, health status and service utilisation data regarding older adults to quantify the scale and scope of future services. 

  • Phase 4: desktop research (models) — we scanned reputable information sources to identify a broad range of delivery models that could be used to shape future phases of the commissioning cycle.

  • Phase 5: service mapping — we mapped all existing relevant programs and services delivered within the Brisbane North PHN region to identify gaps, avoid duplication, and encourage providers to leverage existing infrastructure.

  • Phase 6: reporting and recommendations — we compiled the findings of each of the project phases and submitted a report to Brisbane North PHN on implications and recommendations for next steps.

Vision Alignment: A more impactful social purpose sector

Beacon Strategies enhanced the impact of social services in the Brisbane North PHN region by conducting thorough research and analysis focused on healthy ageing and chronic disease management which identified critical gaps and opportunities for intervention.

This informed the development of targeted initiatives that will support older adults in maintaining independence and managing chronic conditions, while also reducing the strain on local health services. By providing a strategic pathway for Brisbane North PHN to enhance service delivery, we have helped contribute to a better quality of life for older adults.

Outcomes

Our team developed a comphrensive report for Brisbane North PHN, plus a presentation on the summary of the findings and recommendations on potential service offerings based off these findings.
