Background
The Greater Choice for At Home Palliative Care (GCfAHPC) program provided funding for coordinating palliative care through Primary Health Networks (PHNs).
We partnered with Western Queensland PHN to collaboratively design a service mapping tool in order to help them meet community needs for palliative care.
Approach
Our approach to collaboratively developing a service mapping tool with WQPHN followed these steps:
Rapidly analysed situation
To ensure that the service mapping tool met the needs of Western QLD PHN, we rapidly analysed the situation, context and drivers. We asked Western QLD PHN for any relevant documents to support this initial step and engaged with key Western QLD PHN staff to understand the project requirements.
Defined data indicators
We facilitated a series of meetings to develop an agreed set of indicators to guide a consistent approach to our data collection.
Developed a conversation guide
Additionally, we developed a conversation guide to ensure clarity of messaging and consistency in order to support the data collection phase.
Designed a data collection tool
In collaboration with the PHN, we developed a data collection tool to ensure consistent data capture. The data collection tool’s purpose was to collect clean and consistent data.
Briefed PHN team on data collection
Lastly, we prepared guidance and briefed the Western QLD PHN team on how to administer the service mapping data collection tool.
Our team understands the important role that PHNs play at a regional level to increase the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improve coordination of care to ensure patients receive the right care in the right place at the right time.
